Our company is currently in process of getting in network with most major insurance payors. We are actively treating patients with the following insurances. What to bring to your first appointment:

Drivers License & Insurance Cards

Prescription from Referring Provider

Medication List

Any additional health information that is relevant to your therapy services (ie: radiology reports from recent MRI, X-Ray, etc..)


*Paperwork will be emailed to you prior to your appointment for completion, but please arrive 10 minutes prior so that we can ensure everything has been taken care of! 


common faq's

My doctor wrote me a prescription, but it is not on an APT prescription pad. Can you treat me with this? Yes! Just bring it in to your first appointment!


Do I have to have a referral from a physician? We are able to treat you for 8 visits, or 21 days (whichever comes first), without a prescription from a doctor. Please call us to discuss insurance coverage! *We are not able to provide Dry Needling services unless we have a written prescription from a doctor specifically ordering Dry Needling services. 


How can I request Medical Records for myself or a patient? Please email info@arenapt.com or fax the request to 707-736-7250 for Medical Records requests. A consent to release will be required. 


What should I wear to Physical Therapy? Loose fitting clothing is best - something you can move around easily in! 


How much will each session cost? This is dependent on your insurance provider! We are happy to assist with reviewing your insurance benefits for you & all patients will receive financial counseling prior to treatment.
